What is supported living?
Supported living is a service designed to help people with a wide range of support needs retain their independence by being supported in their own home. People in supported living have their own tenancy and are responsible for their own bills and cost of living. This may include full or part furnishing and repairing any damage – exact details will vary locally. To afford these, the person may be entitled to a wide range of benefits and grants. In single person supported living, they will also have their own front door.
In Supported Living, support provision is not dependent on provision of housing (and vice versa.) So if the support provider changes, this doesn’t affect the tenancy. The person has security of tenure in line with their tenancy agreement – they can only be made to leave under certain circumstances, usually by order of a court.

Accommodation
Elmslea offers both private and communal facilities for residents to enjoy. Each resident has their own private bedroom with and en-suite shower room which can be fully or part furnished as required.
All meals are provided and served in the main dining room which is a bright and airy sun room. Residents are able to help with the cooking of meals and encourage to design the weekly menus. There is also a lovely decked area off the main dining room where residents can enjoy alfresco lunches and evening meals together during the summer months.
We have a separate craft room that can be used for individual and group activities from painting and diamond art, to sewing and card making - there is something and space for everyone.
We provide communal laundry facilities for residents to use as part of their weekly household programme.
We are only a stones throw from the local leisure centre, large park and local town with various facilities that residents can also enjoy.

Referrals and Admissions

We encourage referrals from Social Workers, CPN’s and other client representatives and family members.
At first point of contact, pre-visit information and assessments are taken, followed by an arranged half day visit with client and their representative.
At the initial visit, clients will be given a tour and get to meet some of our current residents and staff and be welcomed to enjoy a meal with us.
Following the visit, the proposed resident can consider a trial period to see how life at Elmslea fits with their need. Likewise, we also feel that it is important for other residents to feel comfortable with any new placements.
After a specified trial period, the team from Elmlsea, the new client and their representatives will meet to discuss if a permanent placement would be suitable and meet the needs of the individual.

When a new placement has been confirmed and a move date confirmed, the new resident will be given an introductory tenancy agreement that sets out the terms and conditions relating to their accommodation - support is available to guide the resident through this process.
The initial tenancy agreement will be for a period of 6 month, and following a review at the end of this term, a meeting will be undertaken to confirm a long term tenancy agreement.
This gives both the new residents and the team at Elmslea to ensure that the placement is best meeting the needs of the individual and others living at Elmslea.

As part of the settling in process, each new resident will be allocated a key worker and a buddy to ensure that they have all the support they need to get settled into their new home.
The key worker will meet regularly with the new resident to formulate a weekly activity plan and a long term care and support plan that sets out both short and long term objectives based on the need of the person.
We will also look at what other opportunities are available for the residents outside of Elmslea to support their own hobbies and interests.
Most of all, in the first few weeks, we will do all that it possible to make the new resident feel safe, secure and welcome in their new home.
